Below you will find the link for the ID Badge Renewal Forms for the 2026-2027 Season.
Please complete and SIGN (we cannot accept a typed in signature) both forms and return to our office by either USPS mail or by email (scanned clear copy) to: info@iatselocal500.org
If you have renewed your Driver's License, Passport or Permanent Resident card in the past 6 months, please be sure to include a color copy (front and back) of the new ID.
If you would like to renew your badge in person, please contact the Local 500 office to make an appointment.
